SERPR - New User Application Form

The Strathclyde Electronic Renal Patient Record (SERPR) is the renal system used in the care of renal patients in Greater Glasgow & Clyde, Ayrshire & Arran, Dumfries & Galloway, Lanarkshire and Forth Valley health boards.  Patients with chronic renal failure or patients with complex care needs seen in joint clinics in the other Scottish health boards will also have patient records available in SERPR.
